What is Social Prescribing?

Many things in life can affect your health and wellbeing. Many people visit the GP tell them that they are feeling isolated, lonely or worried about work, housing and money. These are things that cannot be fixed by doctors or medicine alone.

Social Prescribing offers a holistic approach to support patients with “what matters to them” . The idea is to connect you to local community services and support networks to help you have more
control over your health and wellbeing.

How does it work?
Our Social Prescriber will work with you to identify your needs, set goals and signpost you to support within the community, such as:
– Housing or benefits assistance
– Art and creative activities
– Exercise and fitness classes
– Outdoor activities – walks and gardening clubs
– Learning, training, employment or volunteering
– Counselling or therapy groups
– Mental health support groups/ social groups
– Care needs support and advice
… and much, much more

Our social Prescriber is Georgia Coates.
Georgia holds weekly coffee mornings for our patients every Wednesday 11AM-12:30PM at Cedar Barn Stables, Barn Lane, Hazlemere, HP15 7BQ. Coffee Morning Poster
This would be a great opportunity to come and find out about other local groups and services within
the community or even to come and socialise over a nice cuppa and a biscuit. You will always be made to feel welcome.
