Your home address must fall within our catchment area to register as a patient with Desborough or Hazlemere Surgery.
Complete the registration form(s) to register at our surgery. We will ask to see your National Health card, some photo identification – this could be a driving licence, or passport. We will also need proof of your address, this can be a Utility bill or a Bank/Credit card statement. Registrations may take up to 7 days, appointments cannot be made until registration has completed.
You will also be asked to complete a new patient questionnaire. It is very important that we have as much information as possible on your past medical history as it may be some time before your records reach us. We invite all new patients to have a health check with a member of the clinical staff.
At the time of registration every patient is given the opportunity to have a “Summary Care Record” created – please ensure you understand what “SCR” is before choosing whether or not to opt-out. Click here for more information on Summary Care Records:

No fixed abode and registering with a GP in Bucks…
Healthwatch Bucks in conjunction with Wycombe Homeless Connection (WyHC) have recently undertaken a mystery shopper project to find out how easy it is for a person with no fixed abode or who is homeless to register with a GP in Buckinghamshire. Of the 37 practices and 2 branch surgeries contacted, 80% provided a helpful response which is really encouraging.
